Fistula. In plain English. This is usually created in the arm. The cannulla hollow needles are quite large. But the veins in the arm are small. To fit the needles in, a vein needs to be expanded. This is done by shunting (short circuiting) an artery and a vein near the wrist.
Blood flow through the arteries in the arm all the way to the fingertips. Then, comes back through the veins. By joining an artery and a vein near the wrist, blood flows directly from artery to vein. This causes the vein to bulge (expand). That's how a fistula is created.
Some had it in the leg above the knee.
In my case, expansion failed. An artificial fistula had to be created. In front of my elbow fold, a small nylon gortex hose was connected to a vein in 1 end. Then, the other end was inserted inside my arm all the way to near the wrist. Looped back inside and joined with an artery in front of elbow fold.
Still with me. U-shaped and bulging under my skin. Needs surgery to remove. Decided to keep as souvenir.
Temporary access here is using vascap. It's a Y-shaped plastic hose with a hollow needle inserted in one end. Needle is injected into the carotid artery. Dialysis connections done on the 2 other ends. Very uncomfortable.
Pyrogenic reaction. Caused by germs in water. Very rarely happens here. Guess 'coz water used is cleaner. Never happened to me.
Usually starts 2 - 3 hrs after start of dial. Symptoms usually occur in the ff order:
- feeling cold;
- shivering;
- aches & pains all over (like having flu);
- feeling hot;
- elevated temp (could go above 40C.
What to do?
- recirculate blood;
- keep both cannulae flush with saline.
